The Boeing 737 pressurisation system was invented in 1960, for the 727, and has not changed since. It is mostly manual, with bleeds and packs having on-off switches, and no warning system to say the switches are off. The 737 is an ancient aircraft, with 1967 certification, and should not be flying in the 21st century.

Part of the Before Start check list, which the crew failed to carry out correctly. The Greek Helios incident on a 737 a number of years ago caused the aircraft to crash after everyone onboard suffocated through lack of oxygen. A warning horn should alert the crew before the oxygen masks deploy automatically.
